The 2010 Community Needs Assessment and the associated Healthy Living Map includes mapping of the latest health indicator data, zip code level data that can be aggregated and/or compared to county and state rates, and refinements that make the Healthy Living Map much more navigable.  From any page on the Healthy Living Map a user can click on Zip Map to mine health indicator data; Provider Directory to locate health and mental health care services; Data Dictionary for a glossary of terms; and Links for helpful resources pertaining to health care.  We hope you will utilize this information to advance your efforts within the health care community.

For the past twelve years, Catholic Healthcare West member-hospitals (including Mercy), Kaiser Permanente, Sutter Health Sacramento Sierra Region, and UC Davis Health System have partnered on the Community Needs Assessment every three years.  This more efficient and effective collaborative approach provides useful information that supports comprehensive community benefit planning.
